Some children do not notice they are hungry until they are already falling apart. Some do not register a full bladder until it is urgent, or cannot tell whether they are cold, or seem genuinely surprised by their own anger the moment it arrives. It is easy to read this as not paying attention. More often it is a difference in a sense most of us have never been taught to name.
That sense is interoception, and it is the quiet groundwork underneath emotion regulation.
The sense that points inward
Most of the senses we learn about in school point outward. Interoception points in. It is the stream of information arriving from inside the body: hunger, thirst, temperature, the need for the bathroom, tiredness, pain, muscle tension, breathing, a heart that has started to race.
It matters for feelings because feelings are partly physical. A tight chest, a hot face, a clenched jaw, a fluttering stomach. Adults often work backward from those signals without noticing we are doing it. We feel the tightness, we think something like "I am getting wound up," and we act on it early, while acting is still easy. A child who does not catch the signal until it is overwhelming does not get that head start. They go from apparently fine to completely undone, with nothing visible in between.
Seen this way, the puzzling jump from calm to meltdown is often not a jump at all. It is a slow build that nobody, including the child, could see.
What the research actually says, including the messy parts
Interoception has become a popular explanation, and popular explanations tend to get flattened. It is worth being careful here, because the evidence is more interesting than the slogan.
Researchers separate two different things. Interoceptive accuracy is measured by performance, usually by asking someone to count their own heartbeats. Interoceptive sensibility is what a person reports about themselves, how tuned in to their body they believe they are. These two do not always agree, and the gap between them turns out to matter.
A 2016 study found that autistic adults tended to score lower on the heartbeat task while reporting higher subjective sensitivity to bodily sensations, and that the size of the mismatch tracked with anxiety. Being unsure what a body signal means, while feeling it loudly, is its own kind of hard.
A 2025 systematic review pooling 31 studies across the lifespan found a more mixed picture than the popular version suggests. In children and adolescents specifically, five of the seven studies examining cardiac accuracy found no significant difference between autistic and non-autistic groups. Differences show up more consistently in what people report about their own experience than in laboratory tasks.
So the tidy claim that autistic children cannot feel their bodies does not survive contact with the literature. A 2025 interview study with autistic adolescents found the experience running in both directions. Some struggled to notice rising distress until it was too late. Others felt body sensations so vividly that the sensations themselves became frightening, particularly when read as a sign something was wrong.
The practical takeaway is not a score. It is that a child may be getting too little information, too much, or plenty of information they have no words for, and those three situations call for different responses.
Ways to help a child find the words
None of this requires a program or a purchase. It is mostly a way of talking.
Narrate your own body out loud. "My shoulders are up by my ears, I think I need to slow down." You are showing a child that inner states are noticeable, nameable, and ordinary.
Offer words tentatively, never as a quiz. "Your hands are in fists. Is your body feeling tight, or hot, or something else?" A guess a child can reject is an invitation. "How are you feeling?" in the middle of overwhelm is usually one demand too many.
Start with the easy signals. Hunger, thirst, tiredness, and needing the bathroom are more concrete than sadness or frustration, and they build the habit of checking in.
Make a body map together, on paper, in the child's own language. Where does angry live? What does it feel like just before? Their words are the right words, even if they are unusual ones.
Use outside structure while inside signals are still developing. Regular snacks, planned bathroom breaks, and drink reminders are not giving up on interoception. They keep a child steady while it grows.
Remember the too much direction. For a child overwhelmed by internal sensation, more attention to the body can be the wrong move. Reducing input, offering distraction, and calmly explaining that a fast heart is uncomfortable but not dangerous can serve better.
Never argue with a child's report of their own body. If they say they are not hungry or that a sound hurts, that is information about their experience, not a negotiating position.
What is known, and what is not
Structured interoception programs exist, and the evidence for them is early. A 2022 study of a 25 week school based interoception curriculum found statistically significant improvements in both interoceptive awareness and emotion regulation. It also involved 14 students, had no control group, and was not a randomized trial, which its own authors were clear about. That is a promising start rather than a settled finding, and it is worth holding it that way.
The everyday version, the noticing and the naming and the patient vocabulary building, asks nothing of you but attention, and it is where most families can start.
One honest caveat. Tools and companions can support this kind of noticing. They do not do it for a child, and they are no substitute for a caregiver who is paying attention or for professional support when a child's distress is frequent, severe, or affecting daily life. A child who can say "my tummy feels wobbly" has done something genuinely difficult. They noticed. Everything else gets easier from there.
